Group Home Medical Reviews for residents in APD licensed group homes.

New for APD Licensed Group Homes!

Beginning April 17, 2024, Acentra launched the Group Home Medical Review (GHMR) program that provides services on behalf of the Agency for Persons with Disabilities (APD) to individuals residing in APD licensed group homes in the state of Florida. Documentation and reporting will be completely electronic. Group Homes will be provided with a copy of each report.

A Group Home Medical Review is a medical review completed for APD clients residing in an APD Licensed Group Home. A Medical Review includes an onsite observation along with a comprehensive medical record and medication chart review completed by a Florida licensed Registered Nurse, to verify appropriate health care and follow up is occuring timely.

Florida’s Agency for Persons with Disabilities will provide Acentra with a monthly report of group home residents. Acentra will contact the group home to confirm the census and then schedule the medical review to occur on-site. The group home will receive a written notice via mail 30 days prior to the scheduled review. Acentra will conduct the medical review annually to verify the resident’s safety needs are being met and that appropriate healthcare follow-up is occurring timely.

Acentra will request confirmation of the census and complete the reviews. The Acentra Clinical Reviewer will complete the Medical Review forms online. Results will be emailed to the group home contact provided by APD. Please contact Acentra with any questions.
